Ending / Bryan’s Fate

Bryan reaches Raccoon City General Hospital with the vial—what he thinks is a high-priority medical delivery, which turns out to be a special binding agent required to synthesize a cure for the viral outbreak. Throughout the night, he endures terrifying encounters, becomes infected by being bitten by an infected girl, and his infection grows progressively worse.

When Bryan finally gets to the lab and the scientists start working on the cure, it’s too late. His infection has advanced past the threshold where the cure can save him. His body mutates rapidly: tentacles emerge, his torso forms a monstrous maw, and he begins absorbing the scientists nearby. He loses control of himself entirely.

While Bryan transforms, the completed cure slips from the building—falling from a sky bridge multiple stories—it’s never shown if it survives. Bryan, now fully monstrous, lets out a roaring, inhuman cry as National Guard helicopters surround him. The film ends there.
